Terms and Conditions (50$ Deposit)

By making a $50 payment deposit for our service, you agree to the following terms and conditions:

  1. Payment Deposit: You agree to pay a $50deposit to secure your booking for our service. This deposit is required to confirm your reservation and hold your spot.

  2. Non-Refundable Deposit: The $50payment deposit is non-refundable. If you cancel your booking or fail to show up for the scheduled service, you will forfeit the deposit.

  3. Scheduled Service: The scheduled service refers to the date and time for which you have booked our service. You are responsible for arriving on time for the scheduled appointment.

  4. 12-Hour Non-Refundable Payment: The $50 payment deposit will automatically convert to a non-refundable payment 12 hours before the scheduled service. Once this conversion occurs, the payment becomes non-refundable, regardless of any cancellations or changes made thereafter.

  5. Cancellation Policy: If you need to cancel or reschedule your booking, you must do so at least 12 hours before the scheduled service to avoid forfeiting the $50 deposit. Cancellations made within 12 hours of the scheduled service will result in the automatic conversion of the deposit to a non-refundable payment.

  6. Refunds: No refunds will be issued for cancellations made within 12 hours of the scheduled service or for no-shows. The $50 deposit will be retained by us as compensation for the loss of the scheduled appointment.

  7. Changes to Booking: If you need to make changes to your booking, such as rescheduling or modifying the service requested, please contact us at least 12 hours before the scheduled service. Changes made within 12 hours of the scheduled service may not be accommodated and may result in the forfeiture of the $50 deposit.

  8. Exceptions: In the event of extenuating circumstances, such as a documented medical emergency or unavoidable travel disruption, we may consider waiving the non-refundable payment requirement at our discretion. Please contact us as soon as possible in such cases to discuss your options.

  9. Governing Law: These terms and conditions shall be governed by and construed in accordance with the laws of Magi’s Infusions LLC, without regard to its conflict of law provisions.

Terms and Conditions: Agreeing to Intravenous (IV) Therapy

By agreeing to undergo intravenous (IV) therapy, you acknowledge and agree to the following terms and conditions:

1. Understanding of Risks and Complications:

  • You understand that intravenous therapy involves the insertion of a needle into a vein to administer fluids, medications, or nutrients directly into the bloodstream.

  • You acknowledge that while IV therapy is generally safe, it carries inherent risks and potential complications, including but not limited to:

    • Infection at the site of injection

    • Bleeding or bruising

    • Allergic reactions to medications or fluids administered

    • Pain or discomfort during or after the procedure

    • Damage to the vein, including inflammation or thrombosis

    • Rare but serious complications such as air embolism or nerve damage

2. Voluntary Consent:

  • You affirm that your consent to undergo IV therapy is voluntary and based on your own understanding of the procedure, its risks, and potential benefits.

  • You acknowledge that you have had the opportunity to ask questions and seek clarification regarding the procedure and its implications, and that any questions you had have been adequately addressed.

3. Alternative Treatment Options:

  • You understand that alternatives to IV therapy may exist for your condition or treatment goals, including oral medications, topical treatments, or non-invasive procedures.

  • You acknowledge that you have been informed of these alternatives and have chosen IV therapy after considering them.

4. Compliance with Instructions:

  • You agree to comply with all instructions provided by the healthcare provider administering the IV therapy, including pre-treatment preparations and post-treatment care.

  • You understand that failure to follow these instructions may increase the risk of complications and may affect the effectiveness of the treatment.

5. Right to Withdraw Consent:

  • You retain the right to withdraw your consent for IV therapy at any time before or during the procedure.

  • You understand that withdrawing consent may have implications for your health and treatment plan, and you agree to discuss any concerns or decisions to withdraw consent with your healthcare provider.

6. Release of Liability:

  • To the fullest extent permitted by law, you release and discharge the healthcare provider, their affiliates, and staff from any liability for injuries, damages, or losses arising from or related to the IV therapy, except in cases of willful misconduct or gross negligence.

7. Agreement to Terms:

  • By signing or otherwise indicating your agreement to this consent form, you affirm that you have read, understood, and agreed to all the terms and conditions outlined herein.

  • You acknowledge that this consent form serves as documentation of your informed decision to undergo IV therapy.

8. Confidentiality:

  • Any information disclosed as part of your IV therapy, including personal health information, will be kept confidential in accordance with applicable laws and regulations.

9. Governing Law:

  • This consent form shall be governed by and construed in accordance with the laws of [Magis Infusions LLC], without regard to its conflict of law provisions.

10. Modification of Terms:
